Do you take insurance?

We are out-of-network providers, meaning we do not accept insurance. We provide patients with forms they can use to obtain out-of-network benefits through their insurance. If patients wish to find out about their out-of-network benefits, they should call the number on the back of their insurance card. The insurance company will need our address (291 Broadway, Suite 710, NY, NY 10007) and the following CPT codes: 90792 for the first session, and 99213 and 90836 for follow up sessions (they will reimburse for both codes for a single session). To further offset the cost of treatment, many workplaces offer a healthcare flexible spending accounts, which allow patients to use pre-tax dollars for treatment. We accept all major credit cards, flexible spending accounts, and Zelle for payment. We also accept checks for in-person treatment but not for video sessions.

Do you offer video or phone sessions?

We offer video sessions using a HIPPA-compliant version of Zoom. This form of treatment is most appropriate for older adolescents and adults. We also offer phone sessions, although these are normally not reimbursed by insurance.

Do you offer in-person sessions?

We do offer in-person sessions, which is our preferred way of treating children, most adolescents, and many adults. While COVID-19 transmission is a risk, we are fully vaccinated and boosted, encourage patients and caregivers to be as well, and use HEPA air filters to promote safety. We ask that patients mask in the waiting room if others outside of their household are present, monitor symptoms and possible COVID exposures, and refrain from coming into the office if there is a known risk of contagion. We unmask for most sessions because seeing each other’s faces is an essential part of treatment, although we will mask for patient and caregiver preference.

How do I make an appointment?

Call, email, or fill out the form below. You will reach our assistants, Lori and Liz, who will ask some initial details of why you are seeking treatment and set up a time for a brief phone consultation with Drs. Roth or Chouake. We will ask about what brings you to treatment, introduce ourselves and our practice, and answer any questions. Lori or Liz will then send you a link to Luminello, the secure electronic system we use for charting, billing, and communicating with patients. They will ask you to sign forms and upload demographic and billing information. They will then schedule your first appointment.
